Disclosure of medical information

Must be selected in order for registration to be completed.

My child has the following condition(s) and if requested by SWS, has provided a doctor’s note permitting her to participate in dance. All physical, cognitive, and mental conditions must be disclosed. Implications of any condition that may impact on the student’s ability to participate in dancing MUST also be indicated. All conditions MUST be disclosed VERBALLY to the Directors and the child’s personal instructor. The Directors may require a medical certificate prior to the student returning or beginning to train.

I hereby declare that my child is physically fit to participate in dance

I understand it is my responsibility to inform the directors and my child’s personal instructor immediately, verbally and by completing a new Fit to Participate form if there is a change in my child’s physical or mental health not disclosed on this form. Medical documentation may be requested by the Director in order for a dancer to return to training.

I understand that there is a costume deposit due by October 1st of $75.00/class for the year end recital in June. I also have been informed that the deposit is non-refundable and non- transferable and that the costume balance(s) will be due the end of May/beginning of June depending on when the cost is determined. No personal cheques will be accepted for costumes.

By submitting the form, I declare that the information above is accurate and thorough, and I acknowledge that I will/have speak/spoken to the Office Manager and my child’s personal instructor to disclose this information.

Students will be required to sign a recreational waiver within their first week of Fall Classes (Summer Excluded).

​Once submitted an office staff member will contact you to complete the registration payment.
